Dear I/DD community member,
You are being invited to participate in a survey of the intellectual/developmental disability (I/DD) community in Michigan.

We are asking people with developmental disabilities, their family members, service providers and advocates to complete a survey that asks about what is most important to them and the State of Michigan. The purpose of the survey is to help the Michigan Developmental Disabilities Institute (MI-DDI) plan their work for the next five years and to maintain federal funding as Michigan’s University Center for Excellence in Developmental Disabilities. The mission of MI-DDI is to contribute to inclusive communities and quality of life for people with developmental disabilities and their families through education, community support and services, research and communication.

The survey will take about 10 minutes to complete. Please only take the survey once. Your responses do not include your name and are private. At the end of the survey, you will have the option to be entered into a drawing to receive one of six $50 Target gift cards. To keep your survey responses private, the surveys will be kept separate from your contact info for the drawing. We plan to keep the survey open until early December.

In addition, we are also asking that you forward the survey link widely to your networks in the State of Michigan. We want to be as inclusive as possible and hear from people with I/DDs, their families, advocates and providers throughout the entire state. The survey is available online at the link below, over the phone (313-577-0885 or 313-577-9918) or paper surveys can be mailed to you or your organization (please send requests to caitlinmb@wayne.edu). The survey is also available in Spanish or Arabic.
